Thank you for attending the second National Forum for Phase 3 of the Lighthouse Hospital Project. Our goals were to hold an event that was not only a celebration but also informative and practical; providing opportunities for you to share information, discuss sustainability and to celebrate the achievements of Phase 3.
Your attendance, participation, and thoughtful input made the two-day event a great success.
The Lighthouse team at the Heart Foundation thank you for actively participating in this exciting event. We look forward to continue working with all hospitals and their partner organisations until the end of the project.

The Heart Foundation and Australian Healthcare and Hospitals Association acknowledge the Traditional Owners of this land on which we are meeting for the 2019 Lighthouse Hospital Project National Forum – the Wurundjeri People. We would also like to extend a warm and sincere thank you to any descendants from this country attending the National Forum. We would also like to pay our respects to Elders past and present.
On behalf of the Heart Foundation and our project partner, the Australian Healthcare and Hospitals Association, I welcome you to the 2019 Lighthouse Hospital Project National Forum.
The 2019 National Forum brings together each of the 18 hospitals participating in the project, as well as their community partners. This includes Aboriginal Medical Services, Primary Health Networks, and peak bodies involved in improving the health of Aboriginal and Torres Strait Islander peoples.
The 2019 National Forum will provide you with an opportunity to celebrate and learn about the many successful and innovative initiatives implemented through the Lighthouse Hospital Project, and to discuss ways of embedding and sustaining those changes.
I encourage you to reflect on your achievements, to share your learnings and to network and collaborate to improve the heart health of Aboriginal and Torres Strait Islander peoples.
As Phase 3 of the project wraps up, I strongly encourage you to continue the positive momentum that was generated through the project and to continue to influence and push for change. I hope the 2019 National Forum fills you with a sense of achievement, and that you are inspired to share this with your colleagues, your project partners and community.

2018 National Forum Overview
The 2018 National Forum was centred around project priorities which had emerged from workshops, engagement and discussion with project stakeholders and challenges and opportunities experienced across the project sites. The following five themes were developed from these priorities and formed the basis of the National Forum agenda:
Understanding the patient experience
Addressing discharge against medical advice
Increasing patient uptake and engagement with services
Building service capability to provide culturally safe and clinically competent care
Strategies to increase project collaboration
